Obtain a Hit Chain higher than 15.

You will only get a Hit Chain when you attack one or more enemies with the same series of attacks, meaning the Hit Chain won't increase if you hit one enemy a few times, and then another enemy.
You will only get a Hit Chain on a single enemy, so make sure you attack the same enemy to increase the chain. This is easily doable versus the Large Phantoms, or in a mini-boss battle against the enormous phantoms (try the Invincibility power-up if you are struggling).
Note: The Hit Chain text will not appear until you have a 4 Hit Chain.

Fill the Combo Meter all the way to Level 4.

This will eventually unlock during normal gameplay. The Combo Meter fills after killing an enemy, but the process is accelerated if you kill them in different ways. You should be able to unlock this during the first Tokyo mission.

Initiate a Hero Strike Power-Up.

This power-up looks exactly like the icon in the achievement picture. You can get this power-up in two different ways; 1) it can drop in a mission 2) you can buy it from the Upgrade Lab (Inventory). However, it's costy, so I would recommend that you wait for it to drop for you.
Once you get it, simply press D-Pad Up and a Spider-Man/side kick action will be shown, all enemies on screen defeated, and the achievement will be yours.

Fully upgrade Web Line with all available Modifications.

You will need to collect Tech Coins in order to purchase the upgrades. You will get Tech Coins from killing the enemies, the amount of Tech Coins you'll receive is multiplied by your Combo Meter. You will also receive Tech Coins when you complete a level, depending on how many DNA Helixes and Keystone arenas you find, and if you fought a boss. See Fury's Fourteen if you lack Tech Coins.
To purchase upgrades, in the Helicarrier, go left to a computer console, press A when prompted to enter the Upgrade Lab, select Web Lab, and then the appropriate sub section.

All characters are fully upgraded with all Attacks and Attributes.

This achievement involves some grinding, but luckily not that much. First of all, you can use a Tech Code cheat once in order to gain 5,000 Tech Coins, if you so wish. To do this, in the Helicarrier, press UP, UP, DOWN, DOWN, LEFT, RIGHT on your D-Pad. To grind Tech Coins, simply re-play the Temple mission in Nepal, as you will get 3000 Tech Coins for beating the boss.
